Lean from the Trenches describes a project over time and how it was managed.  It devotes a whole chapter to bugs.  I liked the “Visualizing Bugs” section which conveys the explicit policy used to limit bugs.

Kanban Tool has been making some nice improvements recently.  New functionality supports using tags in the email subject line to set properties and add comments.  CFD data can now be exported.  Performance is faster and more color options are available.
